Our Creative Connections Training Center offers engaging opportunities for adults to learn, connect, and grow in an uplifting and supportive environment.  Participants build everyday life skills through engaging activities that encourage independence, confidence, and social interaction.

Events & Community Inclusion

Our training center calendar includes community involved events and outings that promote inclusion, friendships, and a sense of belonging beyond programs and services.

GET OUT AND GROW

This Pathwise Community Involvement Program offers community focused outings, events, and activities to help participants become more involved and confident in their local community.

The program offers opportunities to:

  • Learn more about assistive technology.

  • Apply independent living skills in natural settings.

  • Identify and communicate personal interests.

  • Expand relationships in the community.

  • Explore volunteer and career possibilities

Supported Employment

Career Readiness

Staff assist people in preparing for employment by learning essential workplace skills such as resume building, job searching, problem-solving, completing tasks, understanding schedules and timelines, and interacting appropriately with coworkers and the general public.

Career support

The goal of Supported Employment is to assist people in gaining and retaining competitive employment.  Services include personal coaching for job searching, application processes, interview scheduling, on-the-job training, and other ongoing supports to promote long term success.

Residential

Our Residential Services provide safe, supportive homes in community settings. Supports are personalized to each individual’s needs and focus on building independence while ensuring health and safety.

 

We offer a range of residential options, including:

  • Group homes with varying levels of daily support

  • Smaller homes for individuals who need more personalized care

  • Apartment-style living for individuals who are more independent, with support for things like medication management, finances, and transportation

 

Our goal is always to support individuals in the least restrictive setting while helping them live full, meaningful lives.

Case Management

Case Management Services help individuals and families navigate available supports. Case Managers work closely with families to plan, coordinate, and monitor services, ensuring each person receives the right support at the right time. They also help connect families to additional resources when needed.

Building Dreams Early Intervention

Building Dreams Early Intervention Services supports children ages 0–6 who have developmental delays or special needs. Once a child qualifies, families receive FREE services provided in the home or daycare setting.

Early Interventionists work one-on-one with children to support developmental goals and partner with families to connect them to additional services and resources—helping children grow, learn, and thrive.
